Eve and Gene Black Summer Medical Program

June 30 - August 1, 2014
2, 3 and 4 Week Programs Available
  • Applicants must be in 11th and 12th grade when they fill out an application and have a birth date on or before 7/1/98.  Cedars-Sinai requires student be 18 by start date.
  • The Program is best described as a medical mentor program
  • Under the supervision of a health care educator/medical facility coordinator, selected participants shadow various medical professionals (nurse, nutritionist, pharmacist, lab technician, physician and/or medical specialist ect.) who together provide a range of patient care and services.
  • All involved medical facilities run their programs Monday through Friday for about 8 hours a day, no nights or weekends; the exact hours are determined by the coordinator of each facility.
  • If a financial need exists, a participant may request a stipend.

UC APPLICATIONS

Congratulations to you on the completion of the Fall 2014 UC application!
Although the undergraduate application officially is closed, there still are important steps you as the student need to take after the application.  Here are a few reminders!

  • Print a copy of the application: Applicants should keep a record of the application ID number for reference.
  • Order test score reports: Official ACT and SAT test scores must be submitted by the end of December.
    • Please visit the College Board website to send your scores, as well as ACT website.  Scores can be send to one campus, as test scores are disseminated to all campuses you have applied to.
  • Update the application: Students can log in to the application to review and, if necessary, update their telephone numbers, email addresses, mailing addresses or SAT/ACT scores.
    • If there are changes to your academic record or other parts of the applications, please refer to the website for instructions:
    • After submission of the application, only contact information and/or test score can be updated.  Other changes (personal statement, additional awards, activities, volunteer work, ect) cannot be added.
    • Minor changes to your activities, awards, volunteer work, employment or personal statement are unlikely to have an impact on your admission decision.  However, if you have significant updates in any of these areas, you may send in your changes by email to ucinfo@applyUCsupport.net or by postal mail to UC Application Center, P.O. ELM/EPT

Start smart at California State University (CSU)

CSU requires you to take the English Placement Test (EPT) and the Entry-Level Mathematics Test (ELM) before you enroll. Your scores will determine which math and English courses you should take to begin your career at CSU.

Relax!

The EPT and the ELM are not admission tests. They will not affect your admission to any CSU campus — they simply help determine which courses best match your level of performance in math and English.

Who Takes the Test? Who Doesn't?

Most of you will have to take the exam, but there are some circumstances under which you may be exempt. For example, if you have performed well on another standardized test, such as the SAT® test, or earned transferable credits from another college, you may not need to take one or both of the exams.

Be Early, Be Thorough

CSU test centers fill up on a first-come, first-served basis, so it is best to register as early as possible. Payment will be required at the time of registration and must be made via credit card, e-check or PayPal®. Once you have completed your registration and payment has been verified, an email will be sent confirming your registration and providing a link to your admission ticket. In some cases, a walk-in registration on test day may be possible. The online registration system will indicate which campuses accept walk-in registration. See the 2013 CSU EPT/ELM Information Bulletin (PDF) for more information on walk-in registration.
Note: Only complete registrations, including payment, will reserve a spot for you at the test center on test day.

Items Not Allowed in Testing Rooms

You may not bring a cell phone, smartphone, PDA, calculator, camera, laptop, iPad®, e-reader, pager or other electronic devices into the testing room. You will be dismissed and your scores will be canceled if found with these items.

Fees

The fee for each test is $18, so if you are taking both the EPT and the ELM, the cost will be $36.
The out-of-state testing fee is $50, in addition to the test fees. This fee is non-refundable.
The fee for a duplicate score report is $10. If you took the EPT and/or ELM March 15, 2008 or later, you can access your scores online. The PIN you indicated on your answer sheet will be needed to access this site.
Fees must be paid by credit card, e-check or PayPal. Cash will not be accepted. Fee waivers are not available. Fees are nonrefundable during the two weeks prior to the test date and the additional out-of-state fee is never refundable. All fees are subject to change. Note: By paying by e-check, be aware that you are authorizing ETS at its discretion to use the information on your check to make a one-time electronic debit from your account for the amount of your check; no additional amount will be added. If you do not have sufficient funds in your account, you will be required to pay an additional service fee of $20.

Your Social Security Number

The registration system will ask for a Social Security number. CSU campuses use your Social Security number to identify your student records. International students who do not have a Social Security number should use a taxpayer identification number. Students without a Social Security number or taxpayer identification number should leave this field blank.

Testing with Accommodations

You will be required to register and provide the appropriate documentation for arrangements, either due to a disability or Saturday Sabbath observance, by the registration deadline, which is two weeks before the test date.
Note: If you do not contact the campus Test Office at least two weeks before the test date, you may not get the accommodations you need and you will not get a refund.

Disabilities or Health-related Needs

If you have a disability or health-related need and require testing accommodations, you must contact the Testing Office at the campus where you plan to test to make arrangements before you register. The EPT and ELM are available in Braille, large-print and audio editions.
You will be required to provide the necessary IEP paperwork to the Testing Office you plan to test at. Contact the campus Test Officer for instructions on providing this paperwork. Once you have received directions from the campus Testing Office, complete your online registration and be sure to select the appropriate checkbox for accommodations.

For those that applied to colleges and are taking the December SAT this Saturday.

Don't forget to print out your admissions ticket and take it with you the day of the test!

EOP Deadlines for CSU's

What is EOP? The Educational Opportunity Program is designed to improve access and retention of historically low-income and educationally disadvantaged students. The program provides admission and academic assistance to EOP-eligible undergraduate students. In many cases, the program offers financial assistance to eligible students. Campuses tailor their programs to accommodate the needs of their student population.

Seniors!

Now that you have submitted your college applications make sure you have started to set up your portal accounts for each college.
  • Check your inbox and spam for any emails you may have received from the colleges. Many of you will be given a link in these emails that will tell you how to set up your portal accounts. 
  • It is important that you do this as soon as possible to see if schools have requested any additional paperwork from you.

For those of you that did not receive a college application fee waiver and chose to send a check make sure you have done so!
